## Introduction

Living systems are not random collections of molecules. They are organized at multiple levels: lipids and proteins assemble into membranes, membranes enclose organelles, organelles support cell function, cells interact with neighbors, and groups of cells form tissues. Content Category 2A asks you to connect these levels. On the MCAT, the best answer often comes from linking structure to function: a membrane channel allows selective ion flow; a mitochondrion’s inner membrane supports ATP synthesis; an epithelial tight junction limits paracellular movement.

This subtopic is usually tested through experimental passages. You may be asked to interpret how a drug changes membrane permeability, how a mutation in a cytoskeletal protein affects cell movement, or why a tissue loses barrier function after a junctional protein is disrupted. Build your revision around mechanisms, not isolated definitions.

### Plasma Membrane Structure and Selective Permeability

The plasma membrane is based on a phospholipid bilayer. Each phospholipid has a hydrophilic head and hydrophobic fatty acid tails. In water, phospholipids arrange so the heads face aqueous environments and the tails face inward, away from water. This arrangement creates a barrier to most charged and highly polar molecules.

“Fluid” refers to lateral movement within the membrane. Phospholipids and many proteins can move side-to-side. “Mosaic” refers to the mixture of components embedded in or attached to the bilayer. Membrane proteins may act as channels, carriers, receptors, enzymes, anchors, or recognition molecules. Carbohydrate chains attached to lipids or proteins often face the extracellular side and contribute to cell recognition, immune identification, and adhesion.

Cholesterol is also high yield. In animal membranes, cholesterol reduces membrane fluidity at high temperatures by restraining phospholipid movement, but it prevents tight phospholipid packing at low temperatures. This buffering role helps membranes maintain suitable fluidity across temperature changes.

Membrane permeability depends on size, polarity, and charge:

- Small nonpolar molecules such as oxygen, carbon dioxide, and steroid hormones cross easily.
- Small uncharged polar molecules such as water can cross slowly, but water movement is often aided by aquaporins.
- Large polar molecules such as glucose require transport proteins.
- Ions such as $$Na^+$$, $$K^+$$, $$Ca^{2+}$$, and $$Cl^-$$ generally require channels, carriers, or pumps.

> **Test Tip:**
> If an MCAT question asks which molecule crosses a pure lipid bilayer fastest, choose the smallest and most nonpolar option. Charged molecules are strongly blocked by the hydrophobic membrane interior.

Membranes are asymmetric. The inner and outer leaflets differ in lipid and protein composition. For example, carbohydrate groups usually project outward, where they can interact with extracellular molecules. Loss of normal membrane asymmetry can signal cell damage or apoptosis; for instance, exposure of phosphatidylserine on the outer leaflet is a common marker of programmed cell death.

### Transport Across Membranes and Gradients

Transport can be classified by whether it requires cellular energy and whether movement is down or against a gradient.

Passive transport moves substances down their electrochemical gradient and does not require direct ATP input. Simple diffusion occurs through the lipid bilayer, as with oxygen. Facilitated diffusion uses a membrane protein, such as a channel or carrier, but still moves down the gradient. Glucose entry through GLUT transporters is a classic example of facilitated diffusion.

Osmosis is the movement of water across a semipermeable membrane. Water moves toward the side with higher effective osmolarity, meaning the side with more solute particles that cannot freely cross the membrane. If a cell is placed in a hypertonic solution, water leaves the cell and the cell shrinks. In a hypotonic solution, water enters and the cell swells. In an isotonic solution, there is no net water movement.

Active transport moves substances against a gradient and requires energy. Primary active transport uses ATP directly. The sodium-potassium pump is the major MCAT example. It exports three $$Na^+$$ ions and imports two $$K^+$$ ions per ATP hydrolyzed, helping maintain high extracellular sodium, high intracellular potassium, and a negative resting membrane potential.

Secondary active transport does not directly hydrolyze ATP at the transporter. Instead, it uses the energy stored in an existing ion gradient, commonly the sodium gradient. If sodium moves down its gradient into a cell and drives glucose entry against its gradient, that is secondary active transport. If the two substances move in the same direction, the transporter is a symporter. If they move in opposite directions, it is an antiporter.

> The combined effect of a solute’s concentration gradient and electrical gradient across a membrane.

For ions, concentration alone is not enough. Charge matters. A positively charged ion may be pulled into a negatively charged cell even if its concentration gradient is weak. The electrochemical gradient combines both forces.

Membrane potential is especially relevant in neurons, muscle cells, and epithelial transport. Although detailed action potentials are covered elsewhere, 2A questions may still require basic reasoning about ion movement. If potassium channels open in a typical cell with high intracellular potassium and a negative interior, potassium tends to move out down its concentration gradient. As positive charge leaves, the interior becomes more negative.

> **Exam Warning:**
> Do not treat “down the concentration gradient” and “down the electrochemical gradient” as identical for ions. For charged particles, the electrical gradient can oppose or strengthen the concentration gradient.

Bulk transport uses vesicles rather than channels or carriers. Endocytosis brings material into the cell. Phagocytosis takes in large particles, pinocytosis takes in extracellular fluid, and receptor-mediated endocytosis uses specific receptor-ligand binding. Exocytosis releases material outside the cell, as in neurotransmitter release or secretion of proteins.

### Organelles, Compartmentalization, and the Cytoskeleton

Eukaryotic cells are compartmentalized. Organelles separate incompatible reactions, increase local concentrations of molecules, and create specialized environments. MCAT questions frequently ask which organelle is affected by a toxin, mutation, or experimental manipulation.

The nucleus stores DNA and is surrounded by a double membrane called the nuclear envelope. Nuclear pores regulate traffic between the nucleus and cytoplasm. Transcription occurs in the nucleus, while translation occurs on ribosomes in the cytoplasm or on the rough endoplasmic reticulum.

Ribosomes synthesize proteins. Free ribosomes generally produce proteins that remain in the cytosol, enter the nucleus, enter mitochondria, or enter peroxisomes. Ribosomes bound to rough endoplasmic reticulum produce proteins destined for secretion, membranes, lysosomes, or the endomembrane system.

The rough endoplasmic reticulum is involved in synthesis and initial folding of secreted and membrane proteins. The smooth endoplasmic reticulum synthesizes lipids, participates in detoxification, and stores calcium in some cells. In muscle, specialized smooth ER called sarcoplasmic reticulum releases calcium to trigger contraction.

The Golgi apparatus modifies, sorts, and packages proteins and lipids. A useful sequence for secreted proteins is:

1. Translation begins on a ribosome.
2. A signal sequence directs the ribosome to the rough ER.
3. The protein enters the ER for folding and early modification.
4. Vesicles carry it to the Golgi.
5. The Golgi modifies and sorts it.
6. Secretory vesicles fuse with the plasma membrane.

Mitochondria generate much of the cell’s ATP through aerobic respiration. They have an outer membrane, inner membrane, intermembrane space, and matrix. The inner membrane contains the electron transport chain and ATP synthase. Its folds, called cristae, increase surface area for ATP production. Mitochondria contain their own DNA and ribosomes, supporting the idea that they arose from bacterial ancestors.

Lysosomes contain hydrolytic enzymes that degrade macromolecules, worn-out organelles, and material brought in by endocytosis or phagocytosis. Their acidic internal pH is required for enzyme function. Peroxisomes carry out oxidation reactions and help break down very long-chain fatty acids; they also detoxify hydrogen peroxide using catalase.

> Do not confuse lysosomes with peroxisomes. Lysosomes digest macromolecules in acidic vesicles; peroxisomes carry out oxidative reactions and detoxify peroxide.

The cytoskeleton provides shape, organization, movement, and intracellular transport. It has three main components.

Microfilaments are made of actin. They support cell shape, muscle contraction, cytokinesis, amoeboid movement, and microvilli structure. Microtubules are made of tubulin. They form cilia, flagella, centrioles, the mitotic spindle, and tracks for vesicle transport. Intermediate filaments provide tensile strength and structural support; keratin in epithelial cells is a common example.

Motor proteins move along cytoskeletal tracks. Kinesin and dynein move along microtubules, while myosin moves along actin. In cilia and flagella, dynein-driven sliding of microtubules produces bending motion. In skeletal muscle, myosin interacts with actin to produce contraction.

> When a passage describes failed chromosome separation, think microtubules and the mitotic spindle. When it describes failed cleavage furrow formation, think actin microfilaments.

### Cell Junctions, Extracellular Matrix, and Tissue Organization

Multicellular organisms depend on cells working together. Cells communicate, adhere, form barriers, and interact with extracellular material. These interactions allow tissues to carry out functions that single cells cannot perform alone.

Cell junctions are especially high yield in epithelial tissues. Tight junctions form seals between neighboring cells and limit movement between cells. They help maintain polarity by separating the apical and basolateral membrane domains. For example, intestinal epithelial tight junctions help prevent uncontrolled leakage of gut contents into underlying tissue.

Desmosomes provide strong adhesion between cells. They link to intermediate filaments and resist mechanical stress. They are common in tissues exposed to stretching or friction, such as skin and cardiac muscle. Gap junctions form channels between adjacent cells, allowing ions and small molecules to pass directly from one cell to another. They support electrical and metabolic coupling, especially in cardiac and some smooth muscle.

The extracellular matrix, or ECM, is a network of proteins and carbohydrates outside cells. It provides support, organizes tissues, influences cell behavior, and transmits mechanical signals. Collagen provides tensile strength. Elastin allows stretch and recoil. Proteoglycans attract water and resist compression. Fibronectin and laminin help cells attach to the matrix. Integrins are membrane receptors that connect the ECM to the cytoskeleton and can transmit signals into the cell.

Tissues are groups of similar cells and extracellular materials that work together. The four major tissue classes are epithelial, connective, muscle, and nervous tissue.

Epithelial tissue covers surfaces, lines cavities, and forms glands. It is polarized, with an apical surface facing a lumen or exterior and a basal surface attached to a basement membrane. Epithelia often have strong junctions and limited extracellular matrix. Functions include protection, absorption, secretion, and selective transport. Simple epithelia are one cell layer thick and are suited for exchange or absorption. Stratified epithelia have multiple layers and protect against abrasion.

Connective tissue usually contains fewer cells and more extracellular matrix than epithelium. It supports, binds, protects, and stores energy. Examples include bone, cartilage, adipose tissue, tendons, ligaments, and blood. Bone has mineralized matrix; cartilage resists compression; adipose stores lipids and provides insulation; blood transports cells and dissolved substances.

Muscle tissue is specialized for contraction. Skeletal muscle is voluntary, striated, and multinucleated. Cardiac muscle is striated, involuntary, and connected by intercalated discs that help coordinate contraction. Smooth muscle is nonstriated, involuntary, and found in walls of hollow organs and blood vessels.

Nervous tissue contains neurons and glial cells. Neurons transmit electrical and chemical signals. Glia support, protect, nourish, and insulate neurons. Although detailed neurophysiology is tested in other content areas, 2A may test the idea that tissue function depends on specialized cell structure.

> In tissue questions, look for clues about extracellular matrix. Abundant collagen, elastin, mineral, or proteoglycan suggests connective tissue. Apical-basal polarity and tight junctions suggest epithelium.

A common MCAT reasoning pattern is to connect molecular disruption to tissue-level effects. For example, a mutation in a desmosomal protein may weaken epithelial adhesion and cause blistering. Loss of tight junction proteins may increase paracellular permeability. A defect in collagen synthesis may reduce tensile strength in connective tissue. A toxin that disrupts microtubules may impair cilia, vesicle movement, or chromosome segregation.
