## Medical College Admission Test (MCAT) Syllabus

Subtopic 3B tests the structure and integrative functions of the main organ systems. In the Biological and Biochemical Foundations of Living Systems section, you should expect organ-system questions to appear in passage-based experimental contexts as well as stand-alone questions. The MCAT often asks not just “What does this organ do?” but “How does this organ affect another system when conditions change?”

- Respiratory system: ventilation, gas exchange, partial pressures, CO₂/pH regulation.
- Circulatory system: heart, vessels, blood pressure, capillary exchange, transport.
- Digestive system: ingestion, mechanical and chemical digestion, accessory organs, absorption, defecation.
- Excretory system: nephron function, filtration, reabsorption, secretion, osmolarity, acid–base balance.

## Introduction

Foundational Concept 3 focuses on how organ systems sense internal and external conditions, communicate, and adjust body functions. Subtopic 3B is broad because it includes the main organ systems themselves: respiratory, cardiovascular, digestive, renal, immune, lymphatic, reproductive, muscular, skeletal, and integumentary systems. For the MCAT, the key is to study each organ system as both an anatomical structure and a participant in whole-body regulation.

The circulatory system is a useful organizing theme. Blood transports respiratory gases, absorbed nutrients, hormones, immune cells, heat, ions, water, and metabolic wastes. But circulation does not act alone: lungs load O₂ and remove CO₂, the gut absorbs nutrients, the liver modifies absorbed molecules, kidneys regulate osmolarity and pH, endocrine organs release signals, and muscles and skin adjust heat loss. MCAT passages commonly test these links.

### Transport and Exchange: Respiratory, Circulatory, and Renal Systems

The respiratory system exchanges gases with the external environment. Air travels through conducting passages into the alveoli, where thin epithelial barriers allow diffusion between alveolar air and pulmonary capillary blood. Oxygen diffuses from alveoli into blood because alveolar PO₂ is higher than venous blood PO₂. Carbon dioxide diffuses in the opposite direction because venous blood PCO₂ is higher than alveolar PCO₂.

A high-yield integration point is the connection between ventilation and acid–base balance. CO₂ reacts with water to form carbonic acid, which dissociates into hydrogen ions and bicarbonate:

$$CO_2 + H_2O \rightleftharpoons H_2CO_3 \rightleftharpoons H^+ + HCO_3^-$$

If ventilation decreases, CO₂ accumulates. The reaction shifts right, H⁺ increases, and pH falls: respiratory acidosis. If ventilation increases excessively, CO₂ is blown off, H⁺ falls, and pH rises: respiratory alkalosis. The kidneys can compensate over time by altering H⁺ secretion and bicarbonate handling.

> When an MCAT question changes ventilation, immediately translate it into CO₂ and pH effects. Hypoventilation means CO₂ retention and lower pH; hyperventilation means CO₂ loss and higher pH.

The circulatory system links nearly every organ system. The right side of the heart pumps deoxygenated blood to the lungs through the pulmonary circuit. The left side pumps oxygenated blood to tissues through the systemic circuit. Arteries generally carry blood away from the heart under higher pressure, veins return blood under lower pressure, and capillaries provide the thin exchange surfaces where gases, nutrients, hormones, and wastes move between blood and tissues.

Capillary exchange is shaped by hydrostatic pressure, osmotic pressure, membrane permeability, and lymphatic drainage. Fluid that leaves capillaries but is not reabsorbed enters lymphatic vessels, which eventually return it to venous blood. This explains why lymphatic obstruction can cause edema and why plasma proteins such as albumin help retain fluid in blood.

The excretory system, especially the kidneys, maintains blood volume, osmolarity, ion concentrations, and pH. The functional unit of the kidney is the nephron. Blood is filtered at the glomerulus into Bowman’s capsule. Useful substances such as glucose, amino acids, water, and ions are selectively reabsorbed from the filtrate back into blood. Additional wastes or excess ions can be secreted from blood into the tubule.

Renal function is commonly tested with endocrine control. Antidiuretic hormone increases water reabsorption, helping conserve water and concentrate urine. Aldosterone increases sodium reabsorption and potassium secretion, with water often following sodium. These hormones link the kidneys to blood pressure and fluid balance. The kidneys also contribute to acid–base balance by secreting H⁺ and reabsorbing or generating bicarbonate.

> Do not confuse kidney secretion with endocrine secretion. In nephron physiology, secretion means movement from blood into tubular filtrate. In endocrine physiology, secretion means release of a hormone from a gland or cell.

### Nutrient Processing: Digestive Organs, Accessory Organs, and Metabolic Routing

The digestive system converts complex food molecules into absorbable units. Mechanical digestion physically breaks food into smaller pieces, increasing surface area. Chemical digestion uses enzymes and other chemical agents to break bonds in macromolecules. Polysaccharides become monosaccharides, proteins become amino acids and small peptides, and fats become fatty acids, monoglycerides, and glycerol.

Food passes through the oral cavity, pharynx, esophagus, stomach, small intestine, large intestine, and rectum. The oral cavity begins ingestion and chewing. The stomach stores food, mixes it with acid and enzymes, and begins protein digestion. The duodenum receives chyme from the stomach plus secretions from accessory organs. The pancreas supplies digestive enzymes and bicarbonate-rich fluid. The liver produces bile, and the gallbladder stores and concentrates it. Bile emulsifies fats, increasing surface area for lipase activity.

Most absorption occurs in the small intestine, especially the jejunum and ileum. Amino acids and monosaccharides generally enter capillaries and travel through the hepatic portal system to the liver before reaching the right side of the heart. By contrast, many lipid-soluble products are packaged into chylomicrons, enter lymphatic lacteals, and later join the bloodstream. This routing difference is a frequent MCAT target.

The liver is a major metabolic and detoxification organ. It processes absorbed nutrients, stores glycogen, synthesizes plasma proteins such as albumin and clotting factors, converts ammonia to urea, and produces bile. Liver failure can therefore cause decreased clotting factors, low albumin, impaired bile production, and accumulation of ammonia. In MCAT passages, abnormal liver function may show up as bleeding tendency, edema, jaundice, or altered nitrogen metabolism.

Digestive regulation also involves nervous and endocrine control. Parasympathetic input generally promotes digestion, whereas sympathetic input tends to decrease digestive activity during stress. Hormones help coordinate feeding and digestion: ghrelin increases feeding behavior, cholecystokinin helps stimulate gallbladder contraction and pancreatic enzyme release, and insulin and glucagon coordinate post-meal and fasting glucose handling.

> For absorption questions, ask: “Is the molecule water-soluble or lipid-soluble?” Amino acids and monosaccharides usually go to portal blood; chylomicrons and fat-soluble vitamins enter lacteals.

### Defense, Reproduction, and Surface Protection

The immune and lymphatic systems defend the body and maintain fluid balance. Innate immunity provides rapid, nonspecific defense through barriers, inflammation, phagocytic cells, complement proteins, and natural killer cells. Adaptive immunity is slower on first exposure but more specific; B cells produce antibodies, and T cells coordinate responses or kill infected cells.

The lymphatic system returns excess interstitial fluid to the bloodstream and transports immune cells through lymph nodes, where antigens can be sampled. It also absorbs lipids from the digestive tract through lacteals. This gives the lymphatic system roles in circulation, immunity, and nutrition.

The skin, or integumentary system, is both a barrier and a regulatory surface. It reduces water loss, blocks many pathogens, contributes to innate immune defense, and supports thermoregulation. Sweat evaporation removes heat. Vasodilation of cutaneous blood vessels increases heat loss, while vasoconstriction conserves heat. The skin is also involved in vitamin D synthesis, linking sunlight exposure to calcium balance and skeletal physiology.

Reproduction is also a system-level process. Gametogenesis produces sperm and oocytes. Fertilization creates a zygote, embryonic development establishes tissues and organs, and pregnancy requires maternal-fetal exchange through the placenta. Late in pregnancy, antibodies are actively transported from the pregnant individual to the fetus, providing early immune protection after birth.

Childbirth, or parturition, depends on coordinated uterine smooth muscle contraction. Prostaglandins and oxytocin promote rhythmic contractions. Oxytocin is a classic example of positive feedback: uterine contractions push the fetus against the cervix, cervical stretch promotes more oxytocin release, and oxytocin strengthens contractions until birth ends the loop.

> Most physiological regulation uses negative feedback, not positive feedback. Positive feedback is usually limited to processes with clear stopping points, such as childbirth and blood clotting.

### Movement, Support, and Whole-Body Regulation

The musculoskeletal system is often associated with movement, but its MCAT relevance is broader. Bones protect organs, support body shape, store calcium and phosphate, and house bone marrow, where blood cells develop. Skeletal muscle allows voluntary movement, supports posture, contributes to venous return, and generates heat. Smooth muscle controls lumen diameter and movement in organs such as blood vessels, airways, digestive tract, reproductive tract, and bladder. Cardiac muscle generates the pumping force of the heart.

Skeletal muscle is striated, voluntary, and usually multinucleated. Smooth muscle is nonstriated, involuntary, and found in many organ walls; it can contract without direct neural input in some tissues. Cardiac muscle is striated, involuntary, and contains intercalated discs with gap junctions, allowing coordinated contraction. Striated muscle contraction depends on actin–myosin interactions regulated by troponin and tropomyosin.

Calcium links muscle, bone, kidneys, intestine, and endocrine control. Calcium is needed for muscle contraction, neurotransmitter release, blood clotting, and bone mineralization. Bone acts as a mineral reservoir. The intestine absorbs dietary calcium, kidneys regulate calcium loss, and hormones such as parathyroid hormone and vitamin D help maintain blood calcium levels.

Whole-body physiology often depends on feedback loops. A feedback loop includes a sensor, a control center, and an effector. For example, in thermoregulation, temperature receptors detect deviation from normal body temperature, the hypothalamus integrates the signal, and effectors such as sweat glands, blood vessels, and skeletal muscles respond. Sweating and vasodilation reduce high temperature; shivering and vasoconstriction counter low temperature.

Blood glucose regulation is another classic case. After a meal, insulin promotes glucose uptake and storage, lowering blood glucose. During fasting, glucagon promotes release of stored glucose and production of new glucose, raising blood glucose. Diabetes mellitus illustrates how loss of insulin production or reduced tissue sensitivity to insulin can damage multiple organs, including retina, kidney glomeruli, heart vessels, brain vessels, and peripheral nerves.

The MCAT may combine several systems in one passage. For example, endurance exercise increases skeletal muscle ATP demand. Ventilation rises to increase O₂ uptake and CO₂ removal. Cardiac output increases to deliver O₂ and nutrients. Skin blood flow and sweating increase heat loss. Hormones mobilize glucose and fatty acids. Kidneys conserve water if dehydration develops. A strong answer will connect the altered variable to the organ systems that detect, transport, and correct it.
