The MCAT often tests these ideas by giving a physiological scenario and asking which physical principle explains it.

In one continuous vessel, velocity may increase in a narrowed region even though overall flow rate remains the same.

Real blood is not an ideal fluid: it has viscosity, contains cells, and travels through flexible vessels. Still, MCAT passages often tell you when to use an idealized model. When viscosity matters, Poiseuille’s law is usually the key equation:

Flow may be laminar or turbulent. Laminar flow is smooth, with fluid layers sliding past each other. Turbulent flow is chaotic and can occur at high velocity, low viscosity, large vessel diameter, or after abrupt narrowing. In physiology, turbulent flow may be associated with heart murmurs or abnormal flow through narrowed valves.

### Gas laws, ventilation, and movement of air

Gases are fluids, so they flow in response to pressure differences. They are also compressible, so their pressure, volume, temperature, and amount are related by the ideal gas law:

In the lungs, a key special case is Boyle’s law. When temperature and amount of gas are held constant, pressure and volume are inversely related:

$$P_1V_1=P_2V_2$$

During inhalation, the diaphragm contracts and moves downward. The external intercostal muscles also help expand the thoracic cavity. Thoracic volume increases, intrapulmonary pressure falls below atmospheric pressure, and air flows into the lungs. During passive exhalation, these muscles relax, thoracic volume decreases, intrapulmonary pressure rises, and air exits.

This is one of the simplest ways the MCAT links physics to biology: increasing volume decreases pressure, and gases move from higher pressure to lower pressure.

Temperature must be in kelvin for gas-law calculations. Standard temperature and pressure are often given as 273 K and 1 atm, and one mole of an ideal gas occupies 22.4 L at STP. Real gases deviate most from ideal behavior at high pressure, low volume, and low temperature, when molecular volume and intermolecular attractions become more significant. At high temperature and low pressure, ideal gas behavior is usually a good approximation.

Gas movement can also involve diffusion. Graham’s law states that lighter gases diffuse or effuse faster than heavier gases at the same temperature. Diffusion means spreading from high concentration to low concentration; effusion means movement through a small opening under pressure. In physiology, the most tested gas movement is diffusion across the alveolar-capillary membrane.

Surface tension also matters in alveoli. Water molecules lining alveoli attract each other, creating a tendency for alveoli to collapse. Pulmonary surfactant reduces surface tension, making it easier to inflate alveoli and helping prevent collapse, especially in small alveoli.

Air is a mixture, so oxygen’s effect depends on its partial pressure rather than simply the total atmospheric pressure. At high altitude, total atmospheric pressure is lower, so the partial pressure of oxygen decreases even if the fraction of oxygen in air is similar. This lowers the driving force for oxygen transfer into blood.

Each alveolus is surrounded by capillaries. Deoxygenated blood arrives from the pulmonary arteries after being pumped by the right ventricle. Alveolar walls and capillary walls are very thin, often described as one cell thick, which allows rapid diffusion. Oxygen diffuses from alveolar air into blood because alveolar oxygen partial pressure is higher than venous blood oxygen partial pressure. Carbon dioxide diffuses from blood into alveoli because venous blood carbon dioxide partial pressure is higher than alveolar carbon dioxide partial pressure. Oxygenated blood then returns to the left atrium through pulmonary veins.

No ATP is needed for this gas transfer. Diffusion occurs down partial pressure gradients.

Henry’s law describes how gas dissolves in liquid:

$$[A]=k_HP_A$$

The concentration of dissolved gas $$[A]$$ is directly proportional to its partial pressure $$P_A$$ above the liquid. The constant $$k_H$$ depends on the identity of the gas and the solvent.

This law explains several physiological and medical examples:

Gas exchange can fail through problems with ventilation, perfusion, or diffusion. Ventilation is airflow into and out of alveoli. Perfusion is blood flow through pulmonary capillaries. Diffusion depends on membrane thickness, surface area, and partial pressure gradients. Fibrosis thickens the alveolar-capillary barrier, decreasing diffusion. Emphysema reduces surface area. Pulmonary embolism can reduce perfusion. Airway obstruction can reduce ventilation. All may produce hypoxia, meaning low blood oxygen.

Carbon dioxide also affects blood pH. CO₂ reacts with water to form carbonic acid, which can dissociate into bicarbonate and hydrogen ions:

$$\mathrm{CO_2+H_2O \rightleftharpoons H_2CO_3 \rightleftharpoons H^+ + HCO_3^-}$$

If CO₂ rises, the equilibrium shifts toward more hydrogen ion, lowering pH. Hypoventilation can increase CO₂ and promote respiratory acidosis. Hyperventilation removes too much CO₂ and can raise pH, promoting respiratory alkalosis. The medulla helps regulate breathing in response to CO₂-related changes; prolonged voluntary hypoventilation is eventually overridden as CO₂ rises.

This pH connection is a common MCAT bridge between physics, chemistry, and physiology. A question may begin with ventilation, ask about CO₂, and then test acid-base reasoning using Le Châtelier’s principle.
