## Medical College Admission Test (MCAT) Syllabus

In the Psychological, Social, and Biological Foundations of Behavior section, social processes are tested as part of Foundational Concept 7: behavior and behavior change. This section contains 59 questions in 95 minutes, with content drawn mainly from psychology and sociology. Expect both discrete questions and passage-based questions that ask you to interpret experiments, identify social mechanisms, and apply definitions to patient, classroom, workplace, or community scenarios.

## Introduction

Human behavior is rarely produced by individual motives alone. People change how they act when they are watched, placed in a group, exposed to authority, rewarded or punished by peers, or taught social expectations. MCAT questions on social processes often test whether you can identify the social force acting on the person, rather than assuming the behavior is caused by personality, intelligence, or moral character.

This subtopic is high-yield because social processes connect directly to medicine. Patients may follow a treatment plan because of family norms, refuse care because of peer pressure, fail to act in an emergency because of diffusion of responsibility, or adopt professional behavior through medical socialization. On the exam, the same concept may appear in a lab experiment, a school setting, a workplace, a public health campaign, or a clinical passage.

### Group Presence and Individual Behavior

The mere presence of other people can affect performance. The classic distinction is between simple or well-learned tasks and complex or unfamiliar tasks. When people are being observed, physiological arousal tends to increase. That arousal can improve performance on easy or practiced tasks, but impair performance on difficult or new tasks.

For example, a trained runner may run faster in a race than alone because the crowd and competitors increase arousal. A student who has rehearsed a presentation many times may speak more fluently in front of classmates. However, if the task is new or mentally demanding, the same audience can hurt performance. A novice pianist may make more mistakes at a recital than during practice.

> Do not define social facilitation as “people always perform better in groups.” The MCAT may test the full pattern: performance improves for simple or familiar tasks but can worsen for complex or unfamiliar tasks.

Group presence can also reduce individual effort. This is especially likely when individual contributions are not identifiable.

A student may work less hard on a group presentation if the final grade is shared equally. A person in a tug-of-war may pull less strongly when part of a large team. Social loafing is reduced when individual accountability is high, the task is meaningful, group members are highly committed, or each person’s contribution is visible.

Another group effect is deindividuation, in which people lose self-awareness and self-restraint in group settings. This is more likely when people feel anonymous, aroused, or submerged in a crowd.

Examples include people yelling insults in a large crowd, behaving aggressively while masked, or posting cruel comments from anonymous accounts. Deindividuation does not always lead to antisocial behavior; it can also increase adherence to group norms. If the group norm is generosity, anonymity may increase giving. If the group norm is aggression, anonymity may increase harmful behavior.

Helping behavior is also shaped by group context. The bystander effect describes the finding that people are less likely to help when other bystanders are present.

A major mechanism is diffusion of responsibility.

If one person sees someone collapse, that observer may feel personally responsible to call emergency services. If twenty people see it, each may assume someone else has acted. Bystander intervention is also affected by whether the situation is noticed, interpreted as an emergency, accepted as one’s responsibility, and followed by a decision about how to help.

> In passage questions, ask: “Is the person changing behavior because others are present, because effort is shared, because identity is hidden, or because responsibility is spread across a crowd?” That question separates social facilitation, social loafing, deindividuation, and the bystander effect.

### Conformity, Obedience, Norms, and Social Control

Social groups guide behavior by creating expectations. Some expectations are informal, such as how loudly to speak in a library. Others are formal, such as hospital hygiene rules or laws against smoking in certain areas.

Norms may be descriptive or prescriptive. Descriptive norms describe what people commonly do: “Most students wear casual clothes to lecture.” Prescriptive norms describe what people should do: “Students should not talk during an exam.” Both can influence behavior. Public health campaigns often use norms by saying, for example, that most people in a community are vaccinated or that peers approve of safer driving.

Sanctions can be positive, such as praise, awards, promotions, or social approval. They can also be negative, such as criticism, fines, exclusion, or loss of status. Sanctions may be formal, such as legal penalties, or informal, such as disapproving looks.

Deviance is not the same as immoral behavior. It depends on the group and context. Wearing a swimsuit is normal at a pool but deviant in a lecture hall. A behavior may be deviant in one culture, time period, or institution but accepted in another. MCAT passages may use deviance to discuss stigma, labeling, mental illness, criminalization, or health-related behavior.

Conformity occurs when individuals change behavior to match a group.

Conformity can occur for two main reasons. Normative social influence involves conforming to gain social approval or avoid rejection. Informational social influence involves conforming because others are seen as a source of accurate information, especially in ambiguous situations. For example, a patient may follow a crowd toward a hospital exit during a fire alarm because others seem to know where to go. A teenager may wear a certain style of clothing to avoid being excluded by peers.

Peer pressure is a form of social influence from members of one’s peer group. It is often discussed in adolescence, but it can occur at any age. A resident physician may adopt a department’s communication style, or an adult may alter diet, exercise, or substance use because friends model or reward certain habits.

Obedience is different from conformity. In obedience, the behavior changes in response to a direct command from an authority figure.

Classic obedience studies showed that ordinary people may follow harmful instructions when an authority figure appears legitimate, responsibility is shifted to the authority, the setting seems official, and the person being harmed is psychologically distant. MCAT questions usually test the concept rather than the historical details. In healthcare, obedience can explain why patients follow physician instructions, why staff follow hospital policies, or why harmful actions may occur when authority is not questioned.

Compliance is related but distinct. Compliance is changing behavior in response to a request rather than a direct command. Techniques include foot-in-the-door, door-in-the-face, lowball, and reciprocity, though those are often covered under attitude and behavior change. For 7B, focus on the social influence: people often comply to be liked, avoid conflict, or follow social scripts.

> Conformity, obedience, and compliance are not interchangeable. Conformity involves matching a group; obedience involves an authority’s command; compliance involves agreeing to a request.

### Group Decision-Making: Polarization, Risk, and Groupthink

Groups do not simply average individual opinions. Discussion can shift members toward more extreme positions or produce poor decisions because of pressure for agreement.

If most members of a group initially favor a cautious policy, discussion may make the group even more cautious. If most members initially favor a risky treatment innovation, discussion may make the group more willing to take risks. The key is that the group moves further in the direction it was already leaning.

A related term is risky shift, a specific type of group polarization in which group discussion leads to riskier decisions. Risky shift is not the same as group polarization in general because polarization can also make a group more conservative.

Group polarization can happen because people hear persuasive arguments supporting the dominant view, compare themselves with others, and shift toward what appears to be the valued group position. Modern examples include online communities in which members repeatedly encounter similar opinions and gradually adopt more extreme attitudes.

Groupthink is different. It concerns the quality of group decision-making, especially in cohesive groups that value unity.

Signs of groupthink include illusion of unanimity, self-censorship, pressure on dissenters, rationalization of warnings, belief in the group’s moral correctness, and failure to consider alternatives. A hospital committee might dismiss safety concerns because members do not want to challenge a senior physician. A public health team might ignore data that contradicts its preferred campaign strategy because everyone wants to maintain consensus.

Groupthink is more likely when groups are highly cohesive, isolated from outside opinions, led by a directive leader, under stress, and lacking clear decision-making procedures. It can be reduced by assigning a devil’s advocate, inviting outside experts, encouraging anonymous feedback, and having leaders withhold their preferences at the start of discussion.

> If a passage says group discussion made attitudes more extreme, think group polarization. If it says members suppressed disagreement to preserve unity and made a bad decision, think groupthink.

### Socialization, Roles, and Institutions

Social processes also work over longer periods through socialization: the lifelong learning of norms, values, beliefs, roles, and behaviors.

Socialization begins early. Family is often the first agent of socialization, teaching language, manners, beliefs, gender expectations, health habits, and emotional responses. Schools teach both formal knowledge and a hidden curriculum: punctuality, competition, cooperation, respect for authority, and evaluation by performance. Peers become especially influential in adolescence, shaping clothing, slang, risk-taking, music preferences, and attitudes toward studying or substance use.

Media also socialize behavior by presenting models of beauty, success, violence, health, illness, and social roles. Workplaces teach professional norms, such as punctuality, dress, teamwork, and hierarchy. Religious organizations, sports teams, neighborhoods, and legal systems can also act as agents of socialization.

Medical training is a common MCAT-relevant example. Students learn technical knowledge, but they also learn professional norms: how to speak to patients, how to present cases, how to respond to hierarchy, how to manage emotion, and how to think about responsibility. This is socialization into the physician role.

A person can hold several roles at once: student, sibling, employee, patient, caregiver, athlete. Role conflict occurs when expectations from different roles clash. For example, a medical student may struggle when exam preparation conflicts with family caregiving. Role strain occurs when demands within a single role are hard to meet, such as a resident facing many expectations within the physician-in-training role.

Socialization can involve internalization, in which norms become part of the person’s own beliefs rather than remaining external rules. A child may initially brush teeth because a parent demands it, but later views dental hygiene as personally valuable. A medical trainee may initially wash hands because of supervision, then later internalizes infection-control norms.

Socialization also connects with social control. Once people learn norms, groups maintain them through sanctions. A student who interrupts a lecture may receive disapproving looks; a clinician who violates safety protocols may receive formal discipline. These sanctions reinforce the norms of the setting.

MCAT questions may ask you to connect socialization to health disparities or patient behavior. For example, a patient’s willingness to seek preventive care may be influenced by family beliefs, community norms, prior experiences with institutions, language, stigma, and trust in medical authority. A strong answer avoids blaming the individual and instead identifies the social process affecting behavior.
